About the role
In this role you will work within a high-quality, highly communicative/collaborative, and team-based architecture to oversee the comprehensive health and well-being of patients. Physicians will participate in regular huddles with their team to proactively identify health care issues and orchestrate a plan of care with the assistance of their team and the support of an entire organisation.
What will you be doing in this role?
- Establish and maintain long-term relationships with patients
- Manage chronic and acute illness and injuries
- Track and review normal physical development as well as psychological well-being
- Establish an outstanding relationships with and lead a team comprised of a Registered Nurse and Patient Care Administrator
- Create personalized reports for patients to help them and the team navigate their health goals and care
