What you'll do
Foresight Health is building the world's first proactive AI health system, starting with neurodegenerative disease. Our goal is to fundamentally change how chronic disease is managed, moving from reactive clinic visits to continuous, intelligent care that catches problems before they escalate.
Today, we partner directly with specialty clinics to deliver Chronic Care Management as a fully managed service. We combine our clinical team with AI technology to conduct regular patient phone calls, monitor symptoms, detect early issues, and track disease progression over time.
Providers get extended care beyond the visit, better outcomes for their patients, and Medicare reimbursement. We're a small, fast-growing team working with forward-thinking physicians, and we are building toward something much larger than where we are today.
